  • Provide support, reassurance, and peace of mind for family members and caregivers

  • Minimally weekly phone calls and monthly in-home visits

Hospice Advisement

 
  • One- time consultation

  • Explain Hospice benefit, elements of service, and eligibility requirements

  • Assist with interviewing and choosing hospice provider

  • Assess home setting for necessary medical equipment

  • Discuss possible supplemental care and explore options
