• Ongoing condition assessment and care management for optimal quality of life

• Coordination of in-home care requirements with what is and is not provided by Medicare

• Attendance at critical medical appointments, as well as ongoing communication with medical team

• Facilitation of family communication through in person and virtual meetings

• Manage expectations for patient and family

• Provide support, reassurance, and peace of mind for family members and caregivers

• Minimally weekly phone calls and monthly in-home visits

Hospice Advisement

• One- time consultation

• Explain Hospice benefit, elements of service, and eligibility requirements

• Assist with interviewing and choosing hospice provider

• Assess home setting for necessary medical equipment

• Discuss possible supplemental care and explore options
